ABOUT THIS DOCUMENT:
This Affidavit is sworn by both parents. It is required by Immigration Officer when a child is travelling unaccompanied.
REQUIREMENTS:
• Name and age of minor
• Name, ID and telephone of the person to receive the child at the port of destination.
PROCESS:
• Fill in this affidavit.
• Sign before a Commissioner for oaths, Notary Public, Magistrate or Consular officer.
The child to carry 2 originals during travel and to present to immigration officer if required.
