This affidavit is required by Kenya National Examination Council (KNEC) when applying for replacement of a lost exam Certificate.

• Fill in this affidavit.
• Sign before a Commissioner for oaths, Notary Public, Magistrate or Consular officer.
• Report the loss or damage to police. Obtain a police abstract.
• Apply for a replacement on KNEC portal.
