ABOUT THIS DOCUMENT
This affidavit and Form CR7 require to be filed with Registrar of Companies to update change
of Director’s particulars e.g email, phone number, address etc.
REQUIREMENTS
• Name and address of Director.
• Name of Company and Company number.
• Particulars of change eg email, phone number, address etc.
• Name and address of the person to lodge this application with the Registrar e.g
director, company secretary, agent etc.
PROCESS
• Fill in this form.
• The affidavit is sworn before a Commissioner for oaths, Notary Public, Magistrate or
Consular officer.
• Upload the affidavit and form CR7 (Notice for Change of Particulars) on the Business
Registration Services portal on e-citizen.
